Gastric Bypass for Morbid Obesity and Conversion of Dilated Vertical Banded Gastroplasty to Gastric Bypass, Ciné Clinic

A technique of divided, vertical pouch, Roux-en-Y gastric bypass is shown. The first patient had complications of sleep apnea, reflux esophagitis, hypertension, and severe back pain. An uncomplicated gastric bypass was performed. The second patient had a vertical banded gastroplasty years ago, but experienced constant severe heartburn recently. A conversion to divided gastric bypass was performed when a markedly dilated gastroplasty pouch with free esophageal reflux was found. Polyps, malignant pedunculated polyps, and large benign sessile lesions were found; the latter utilizing the piecemeal polypectomy technique devised by Shinya.

 

Laparoscopic Isolated Gastric Bypass After Failed Bariatric Surgery

We present the case of a patient that had previous bariatric surgery. She developed a gastro-gastric fistula after conventional gastric bypass (not isolated) which led her to regain the weight she had lost before (BMI 36). Laparoscopically, we divided and resized her gastric pouch and performed a Roux-en-Y reconstruction. The surgery and postoperative outcomes were uneventful. Three months after surgery, the patient lost 14kb. (BMI 30).

 

Roux-en-Y Gastric Bypass with Triple-Stapled Gastric Pouch

This video describes our triple-stapled gastric bypass procedure for morbid obesity. The senior author has performed over 1,000 of these procedures. Using this technique, the staple line break-down rate is <2%. Weight loss at 1 to 3 years is 2/3 of excess weight, 60% at 5 years, and mid-50's years 6-9. Mortality is <.055, and anastomotic leak rate is 2.5%

 

Laparoscopic Roux-en-Y Gastric Bypass
